CMHI-paid for by Indiana Department of Child Services (DCS) as part of the "Child Mental Health Initiative" It provides family with a wraparound facilitator who facilitates the family in building a natural support system for their child.
The people identified for services; it can be a child and one parent, child and two parents, child and grandparent, child with multiple caregivers, etc. They need to be at a level of need of 4 or above with a score of 1 in the DMHA/DCS project on their CANS.
Who is the "youth and family"
They receive increased support through the WF, team building, and increased skills and services paid on their behalf.
What's the difference between Wraparound and what they already have?
This is wraparound for kids with Medicaid for insurance
CMHW: Child Mental Health Wraparound
This provider coordinates with every one on the team on a weekly basis and facilitates team meetings for the family every month. They make sure everyone stays on the same page, is accountable the strategies identified during the team meetings, and helps identify barriers.
What is the Wraparound Facilitator's role?
One family can receive a HAB, FST, and the Respite services through their programming, and the other gains the services kids with MRO packages receive like home and community based therapy and skills for the individual and family.
What's the difference between CMHW and CMHI?
This is the family goal that we want to reach by the end of the wraparound program
What is a Family Vision?
This service provider is interviewed and chosen by the family to work with the youth involved in services as a home and community based skills provider; they can work with the youth 2-6 hours a week.
What is a Habilitation Provider or HAB?
Increasing the family's natural support system helps increase the family's resiliency and sense of autonomy; it also decreases feelings of overwhelm for the family and fits within their culture and values that they will continue after wraparound ends.
Why do we focus on the natural supports first and don't get to have the HAB, FST, Respite right away?
The driving force of the behaviors we are seeing. If we know and meet this, then the behaviors will be best explained and reduced.
What is an Underlying Need?
This service provider is interviewed and chosen by the family to train the unpaid caregivers of the youth in services. They can also work with the entire family together on family skills.
What is a Family Support Trainer (FST)?
Why does it take so long to get started?
This is the last phase of wraparound; it can be anywhere from an immediate action step to a 6 month plan of reducing "services" and developing community and natural supports. Once we settle into the Plan Implementation Phase, and behaviors are reduced, we discuss a plan for transitioning successfully.
What is Transition?
A service that is meant to give a break to the caregiver, or in the event of a parental crisis where the youth needs immediate care (hospitalizations or otherwise). There are multiple different billing codes for this service.
What is Respite?
These are the first things the family noticed and experienced for their child. Like the difference between this child and their other children, the first time others noticed the behavior (like school professionals or family), and the first time the family had contact with mental health or DCS.
What are Initial Conditions?
