You use this screener for all youth 6 or 7 years old with typical development with known history or clinical reason to screen.

•Runaway/unknown location of youth

•Sexual contact that conflict with the law/PSB
•Death/serious injury to youth or immediate family member
•CPS report/Suspicion of abuse/neglect

•Any ER treatment for a medically fragile youth
•New access to weapons that pose high safety risk
•Physical or verbal threats made towards the Specialist that may result in imminent harm or create a significant safety risk

Interrupted Attempt
List a critical event that has to be reported after session to supervisor.
•Physical altercation that resulted in injury
• Continued access to weapons that pose high safety risk INCLUDING weapons that are left unlocked
• Circumstances that create immediate risk for placement disruption
•Intoxication by a primary caregiver during session that does not result in abuse/neglect of children in their care
•Illegal drugs or drug paraphernalia found in the home
•Failed drug screens by the youth
•Caregiver demand for immediate removal of youth from the home; imminent instability in placement
•Hospitalization or Emergency Medical Treatment of a family member
•Vehicular accident involving the youth or youth’s caregiver that did not result in serious injury
•Out of school suspension/expulsion
•Gang activity that doesn’t fall into a category above
