Do we still record DAP# for pediatric cases and where would I find the DAP number in each room? What is the age range for pediatric cases?
Yes, we still record the cumulative DAP# for pediatric cases, ages 0-16. The DAP number in room 1 is found on the console after an exposure. In Room 2 the DAP number is listed in the picture info beneath an xray image
What should you do when you are not actively working on your computer to ensure patient confidentiality?
Minimize screen, make sure patient info isn’t easily visible.
What should you do if you have a STAT finding after 4pm?
Call No.3 Road Rad. Have front staff burn images on CD, give pt physical copy of report before leaving if they need to go to ER
Explain the company’s pregnancy and lead policy - what ages and exams and if a patient is pregnant but wants to continue with the xray exam what are the necessary steps? (show me where to find form)
Inquire if there is a chance for pregnancy between the ages of 11-55 years for all exams. If the patient is pregnant and still wants xray, call and check with either ordering GP or our Rad for approval (make sure you note in RIS Dr’s name and approval) to proceed with the exam. Then have the patient read and sign the pregnancy/xray consent form and ask reception to scan signed form into their file

Deal with injury as necessary first, submit a written report to andrie as well as incident report , Andrie or Karen to submit report to worksafeBC. Injured staff must also inform worksafe BC. In red safety binder.
