Either subduction (Convergent) boundaries or sea-floor spreading (Divergent) boundaries. Magma begins to spread through the asthenosphere looking for an exit in the lithosphere. Breaks in the boundaries create vents or escape routes for the magma. A volcano is formed where a large underground pocket of magma is located.
Pressure builds under the volcano top until it explodes outwards causing volcanos to blow their tops.

The layers go from coolest to Hottest. The inner core is the hottest layer of the earth but is in a solid state of matter because of the pressure from the other layers of the earth.
The inner core is still very hot but it has less pressure and is a liquid.
The Lower Mantle and upper mantle are bother liquids but are a little cooler and so they move like hot plastic.
The crust is a solid state of matter and is less dense than the mantle, So it floats on top.

It can come from volcanic eruptions, underwater landslides and underwater earthquakes.
Energy is released from the disaster into the ocean water causing it to rise above the normal ocean level. It is then pushed down again by gravity creating trough's and crests in the water. This water is then displaced at a high speed and races across the ocean towards the land.
