What are the 3 phrases we should use when asking for a familiar voice? 

1. I would like you to know that (HCP name) requires that you bring someone with you to the appointment. Who will be joining you for the appointment.

2.  As part of the evaluation, (HCP name) will not only be assessing how well you hear, but also how you understand speech. For this part of the evaluation, we will involve the person whose voice you are most familiar with. Whose voice are you most familiar with?

3. (Patient name), during the evaluation, (HCP name) will be providing you with a great deal of information. Our Patients have commented about how helpful it is to have someone with them. Is there someone else whose voice you are most familiar with?
