It is specific, purposeful and is not provided to all individuals that may be seen in or outside a day program, shelter, or other homeless facility/agency.  The purpose of outreach is to identify those individuals potentially meeting PATH eligibility who are living outside and engage in a conversation to determine PATH eligibility.  Outreach is face-to-face with the individual. Telephone contact is not counted as outreach.

The service note describes all services as specified on the HOPE/PATH Plan as well as any issues or events with the individual. Outreach contacts are not documented in the HOPE/PATH Record.
