IR: incident report (when a client is injured)
DR: discovery report (same form as an IR, but when there was no injury, we are just documenting something we noticed)
CR: crisis report (when a client aggresses or injures another client)
TIR: teammate incident report (when a teammate is injured while at work)

NEVER let a client stay wet or dirty longer than the few minutes it takes to get them to the restroom!
