UFUF ensures confidentiality for all participants by offering the following forms
The Illinois Birth to Three Institute Confidentiality Form (done at intake and updated yearly)
Participant Consent Form: Video Tapes of Parent-Interaction(done at intake and updated yearly)
Ounce of Prevention Fund Release Form: Photography, Video, and Public statement (done at intake and update yearly)
Participant’s Rights Form (done at intake and update yearly)
All family files are stored in locked file cabinets.
FSS discusses information related to families only with site team, supervisors, and Program Director.
The FSS cannot discuss families with an outside provider unless Consent for the Release of Information Form has been signed.
FSS follows procedure when he/she suspects child abuse/neglect
Immediately speak with his/her supervisor or program director. If imminent danger is threatened, staff will call 911 prior to calling supervisor.
Contact the child welfare hotline immediately after discussion with supervisor
The staff report must be done immediately and Confirm report within 48 hours.
