No. If a patient needs to schedule an appointment with General Surgery, the Lab Front Desk is responsible for scheduling the appointment. Do not redirect the patient to the third floor.

Which forms should you give the patient when checking in for a Medicare Wellness Exam?
Check the Assigned Patient Questionnaire first. If the patient did not complete the questionnaire through MyChart, provide the pink AWV form.
Do we need to scan Medi-Cal eligibility for flu vaccine?
Yes, Medi-Cal eligibility must be scanned at the time of check-in. If the eligibility documentation is not scanned into the patient’s chart, the clinical team will not be able to administer the vaccine.
For an uninsured pregnant patient, is a $100 deposit required at the time of service?
No. We do not collect the $100 deposit because we have an OB contract in place for uninsured patients.
