What is Read-Back?(closed-loop communication)
The printed label should always be placed on here for specimen.
What is the actual container?
These must be leak proof and puncture resistant and will hold specimen fully immersed with any additives.
What is containers/collection devices?
On the sterile field keep specimen not dry but ______.
What is moist?
Before going on break this should occur to review the specimens that have been collected and anticipate being collected.
What is handoff?
Specimen label should include at least two of these.
What are patient identifiers?
When using this aqueous liquid the lid must be on tight, specimen must be fully immersed, and proper PPE should be used.
What is formalin?
The specimen will be protected on the sterile field from these things occurring.

These are the 3 F words I will ask about when verbally confirming a specimen
What is fresh, frozen, formalin?
Source, Type and Tests are all included in this.
What are the order requirements for specimen?
These people are involved in the transferring of the specimen.
What is Surgical Tech and Nurse (minimal amount of people)
Never place the specimen on a sponge that has been a part of this activity.
What is a counted sponge?
This will be done with the surgeon before transferring the specimen?
What is verbally confirming?
You will do this immediately upon receiving the specimen and will only do one at a time.
What is Labeling the specimen?
All containers are placed in these.
What are biohazard bags?
If the surgeon uses a syringe to collect a specimen this will be placed on the tip of the syringe.

You will verify this on the specimen label, pathology requisitions form and in EPIC record.
What is patient and specimen identification?
These are the 3 different collection devices for specimen collection.
Sterile cups, formalin jars, Culture swabs
If multiple specimens are collected the Scrub Tech should have this identified area.
