ALL programs complete this on admission on the population selected. May be all clients are a specific targeted population (ex: adults over 55, clients on suboxone, etc).
Timing of follow-up depends on program:
Weekly, every 90 days, or every 6 Months
The time frame from when a service is delivered and a note must be completed
