You have a client that you have not been able to get in touch with. What do you do?
Reach out 2 times a week for 30 days. Contact all available forms of contact after 2 weeks of no contact/response. After 30 days, discharge the client for no contact.
What forms are completed during 12 month updates?

Sometimes there can be only 1 if the client does not have a lot of goal areas to focus on. Most of the time we can make it into 2.
There can be 3 with CPSS goal.

You have one of your clients therapist's emailing you asking to meet to discuss your mutual client. What do you do?
1. Check to see if there is an ROI in the clients chart. If there is not, reach out to the client asking if they would like to create an ROI for you and their therapist to talk.
2. If they have an ROI in the chart, then you can set up a meeting. Once you have the meeting, you will bill for community and fill out a community intervention note.
What goes in domain 1 functioning box?
Copy and paste the diagnostic impression from the most recent DA in the clients chart.
What is the structure of the treatment plan?
1 rehab goal has 2 objectives and each objective has a minimum of 2 interventions.
These are filled out every session, including paperwork sessions. Normally these are completed concurrently with sessions.

How many f2f hours are expected for a full time practitioner?

You have a client tell you that they are having some suicidal ideation in session. What steps do you take?
1. Review their safety plan with them that you created.
2. Call the crisis line together.
3. Follow instruction from the crisis line for next steps (hospitalization, safer environment, verbal contract until next session, etc.)
3. Create a collateral note and send it to supervisor
4. Fill out the SRA at home after the session and note the actions taken in your progress note.
5. Follow through on the plan you created with the client.

If a client has a guardian, how does that change the ITP process?
If the guardian is present during the ITP session, you will add them to the top of the forms in the "individuals involved" section.
You will need to get the guardian's signature in addition to the client's signature.
These are due every 3 months and are completed during group supervisions.
