Go back to the selected medication in the med log. mark that you administered the prn and chart the time the medication was administered. Call the nurse on call if you need help doing so.

It's been a stressful day, and you have a really bad headache, "the only thing you want to do is clock out and go home". Your relief shows up and you have to count Suboxone. Your relief says to you, " Since I'm where both the same times tomorrow, do you want me to count the Suboxone so you can leave now, especially since you're not feeling well. What should you do?
You can thank your relief for being concerned and willing to help you out, but it's policy, I will complete the count with you.
Patient comes to the RA office and states, "I have a really bad headache, and it also feels like I'm going to pass out. RA ask the patient to have a sit in the chair, suddenly the patient vomits. RA takes the patients' blood pressure, and it reads 198/166. What should you do next?
Call the nurse on call, explain report the patient's blood pressure and symptoms the patient is experiencing and wait for further instructions.

It never ok to chart an incident two days later, but if it happens you should report it to your manager and wait for further instructions. Do not get others involved.

RA came into an issue with a patient's medication. You call the nurse on call phone at 10:00pm and you realize it's 10:25pm and the nurse has not returned your call yet. What do you do?
You should acknowledge the time is 10:00pm and for some 10pm is late. You should at least wait until 10:30pm. You may retry your call but if still no answer and it's not an emergency, explain to the patient that you were unable to make contact with anyone and we will follow up in the morning.
If a patient is in pain and they do not have an order for any pain medication. What should you do?
It's never ok to let a patient be in preventable pain. Check to see if the house has a supply of Ibuprofen on Tylenol. Call the nurse on call to advise.

No, you never offer prn medications at med pass time, if a patient wants a prn and if they're able verbally ask then they should ask.

A patient comes to the RA office and states, I forgot to take my hydroxyzine when I took my noon medications. The RA look at the orders and notice, hydroxyzine is not scheduled it's a prn. What do you do?
Explain to the patient that their hydroxyzine is a prn medication, and it will only be administered when ask for it.
Call nurse on call
It's 11pm, the patient fell asleep early that night, he missed his scheduled 9:30pm medications, but decides he want to take it now at 11pm. What do you do?
RA should explain to the patient that medication administration time has a window that allows them to take their medication 1 hour before or 1 hour after the schedule time. RA should call the nurse on call to get approval to administer the medication.
Do not allow the patient to take 1/2 film because that's not what the order states. Patient either takes what's ordered or nothing at all.
