Wire rope guardrail system had a top rail that deflected to 37" high
Per standard 1926 502 b 4, shall not deflect less than 39".
Abatement: Tighten to meet 200# minimum
Gasoline and diesel fuel were stored in containers that were not marked or labeled.
Abatement: Properly label all containers with Product identifier
Temporary electrical outlets were not GFCI protected on a Non-Assured Equipment Grounding Conductor Program job site.
Per 1926.404 b 1 ii Install GFCI's
Abatement: Install GFCI's
Employer did not ensure that a competent person was designated and had completed regular and frequent inspections of the job site, materials and equipment.
Per 1926 20 b 2 Comp Person should provide regular inspections of job sites etc.
Abatement: Competent Person required to do daily inspections.
Eye of wire rope sling was formed using wire rope clips
Per 1926 251 c 4 iii, Eyes in wire rope slings shall not be formed by wire rope clips
Abatement: Eyes should be formed using eye splices or swaged eyes
