Incident: An associate approached to clamp a bundle, honked a couple of times but didn't notice another associate. As the driver clamped the bundle he also clamped the associate foot. What Corrective Action was put in place to prevent re-occurence?
Associates were trained on alternating areas of bundling. This keeps associates on opposite sides of the equipment

Incident: Associate pulls up to the traffic office area and attempts to jump out of the reach truck prior to it coming to a complete stop. While the associates foot is exposed the safety door is pushed by the guard crushing the associates foot. What Corrective Action was put in place to prevent re-occurrence?
Associates have all been reminded during shift start to allow the equipment to come to a full and complete stop before exiting. This information has been added to the forklift training and the written test.
