These two things are protected when restraints are used in addition to preserving his/her dignity, rights and well-being.

What are health and safety?

This is any method (chemical or physical) of restricting an individual’s freedom of movement, including seclusion, physical activity, or normal access to his or her body that (1) is not a usual and customary part of a medical diagnostic or treatment procedure to which the individual or his or her legal representative has consented, (2) is not indicated to treat the individual’s medical condition or symptoms, or (3) does not promote the individual's independent functioning. 

What are restraints?

This person is responsible to ascertain that all providers document time limited orders according to policy, enter all orders in CPRS within the time frame expected according to the type of restraint, participate in a post debriefing about the restraint episode for behavioral health reasons in all areas throughout FHCC, and evaluate the patient for whom they are responsible or assigned.

Who is the Chief Medical Executive?

This is used when the patient’s behavior demonstrates the potential for injury to self or others, or there is a risk of serious disruption of the therapeutic environment and alternative measures have failed.

What is the least restrictive restraint?

This should never be used to discipline patients or residents, as a convenience to staff, or to prevent a patient or resident from wandering.

These staff members write time-limited orders, re-evaluate patients in restraints for whom they are responsible according to the type and reason for restraint used, and participate in post debriefing of restraint use for their patients.

These are time limited and behavior specific. When a restraint is initiated by an RN in an emergency, these must be obtained verbally or written from the physician.

This is any clinical intervention aimed at controlling a patient’s behavior without the use of physical or mechanical devices.

These staff members are responsible to ascertain that the restraint order is in compliance with hospital policy, correctly transcribe time limited-limited orders for restraint use, demonstrate competency in the application and removal of restraining devices, monitor and reevaluate the patient, notify family (identified by the patient), surrogate or next of kin of restraint use, notify nurse manager/nurse coordinator of any instance in which a patient remains in restraint for behavioral health purposes for more than 12 hours, or experiences two or more separate episodes of restraint of any duration within 12 hours and thereafter every 24 hours if either of these conditions continues, document assessment, patient/family education and plan of care, participate in annual review and complete the restraint competency.
