Explain the attestation process.

Both the statewide and nationwide plans provide coverage for out of state coverage dependents. The nationwide plan already provides this coverage without any additional steps. However, dependents on the statewide plans will require an attestation form be submitted before they will have access to coverage outside of the state.

If the participant’s dependent or spouse has an address in the state of Texas, they would not qualify for the attestation process. The home address must be outside the state of Texas to qualify.

If the participant themselves lives outside the state of Texas and are enrolled in ActiveCare, their PCP must be in the state of Texas.
