The person chooses what services they want and who provides those services through a budget.

This Grant Program offers reimbursement under OPWDD guidelines up to $1500 a year for services such as recreation/respite.

A care manager is requested to add in Pre-Voc services to a published life plan.
What is an addendum?
This grant program helps adult with I/DD who live on their own to help with rent and utilities monthly.

A CM has made multiple attempts to hold a life plan meeting and the individual/family has not attended after 3 scheduled attempts. The CM now sends this document home.
What is a life plan compliance letter?
This is a committee who reviews documents for reimbursement or purchasing items on an emergency basis after all other options have been explored.
