How do I document my observation of speech?

Under the results outcome: 

1.  The sounds are modeled for student and he/she continues to say the sound in error.  

2.  During sound discrimination activities, the student is unable to discriminate the correct vs incorrect sound.

3. Student is difficult to understand when asking and answering questions due to the sound sound in error.  He/She is understood 70% of the time in the classroom.
