When is it appropriate to select "Unknown" as a response to a question in CommCare?

Only select unknown if the case was asked the question and they specifically stated that they did not know. DO NOT select unknown if the case was not asked the question, or if they declined to answer. If a case declines to answer a question, you need to specifically state “Case declined to answer xyz questions (ie: medical questions)” in your case note (& free response box if available). If you are unable to ask a question before the interview ends, state in your note “I was unable to ask *write questions or question category here* before the interview was completed”.

Does this statement satisfy the educational requirements for Shortened Case Investigations? Why or why not?

"Provided isolation education. Case educated on warning signs of Covid-19 and when to seek emergency care. Case notified all contacts of positive test result"

No.

This statement correctly documents that the CI provided education for isolation and seeking emergency care, but does not document that education was provided for notifying contacts.

Case's can notify contacts of their positive test result without being informed on the protocols that the contact should follow.

What do you need to do when:

3) A case does have close contacts and agrees to provide you with their information

Declined: "Case declined to provide close contact's information"

No Contacts: "Case reported having no close contacts".

Contacts: "Case's close contacts are as follows: ....."

What should be discussed when educating a case on properly notifying contacts?
