Ask if they would like to us to call family or 911 if they are unable to get up by themselves. We cannot help assist them.

Caregivers- Call the community med pass phone, 2 hours prior to the start of your shift. NO TEXTING! Community staff to assist with coverage if replacement is not found, prior to calling on call.
Dining, Maintenance, LEC's, and Concierge- Call your supervisor 2 hours prior to your shift and assist in finding your replacement!
What do you do if a family member has a complaint?
Ask them if they would like to fill out a Grievance Form.
Where are the Emergency Bins located and what is inside of them?
Laundry rooms.
What do you do if you miss a punch or if the timeclock is not working?
Fill out a missing punch form.
You encounter a resident who had a fall
If Independent- Call 911 or ask the resident if they would like us to call a family member for assistance.
If Assisted - call a nurse during regular business hours. After hours- call triage
A resident is missing....what do we do?
1. Check the Resident Sign-In/Sign-Out Log, 24-Hour Shift Log, and leave of absence status to determine if the resident is out of the Community
2. Notify their supervisor, who in turn will notify the ED
3. Coordinate a search the Community, including resident rooms, bathrooms, closets, and all outdoor areas.
If still not found within the community.
1. Assign a team member to conduct a search of the immediate neighborhood in each direction by car
2. Escalate to area/regional/district leadership
3. Contact the local police department and report the missing resident;
If you are sending a resident out to the hospital- what needs to be sent with them?

How often should you be logging into UKG?
At least once a month. Everyone has annual trainings assigned to them that needs to be completed. If you do not log in on a regular basis, your account will go inactive- and that cause the BOM to have a huge headache! Remember...you get paid to do these trainings!
Training computers are located on 2nd floor, 4th floor, and laptops in Amy's office. Password to get onto computer is Train 123

If you are injured during work- what do you do?
Work comp forms must be filled out BEFORE you leave your shift as well as contact your supervisor.
