What is Point Person?
This amount of time is required for Abuse allegations to be recorded in EIM

This amount of time is required for Neglect allegations to be recorded in EIM

This amount of time is required for Medication Errors in EIM?
What is 72 hours?
What is finalization?
Any person who witnesses or experiences the incident
What is the Initial Reporter?
An act which causes or may cause physical injury to an individual, such as striking or hitting
What is Physical Abuse?
The failure to provide attention and supervision, including leaving individuals unattended.
What is Failure to Provide Needed Supervision?
Individual is given a medication that the individual is not prescribed or has been discontinued
What is wrong, Wrong Medication?
This a recorded in EIM/HCSIS and marks when the incident was observed/reported.
Person who is trained and certified to conduct investigations
What is a Certified Investigator?
An act which causes or may cause mental or emotional anguish by threat, intimidation, humiliation, isolation, or other verbal or nonverbal conduct to diminish another
What is psychological abuse
An event that may cause harm or lead to inappropriate medication use while the medication is in the control of the person(s) charged with administration.
What is Failure to Provide Medication Management?
Individual is given too much or too little medication during a scheduled administration.
What is, Wrong Dose?
Member is a person designated by a provider, SCO, or AE to participate in the administrative review process
What is the Administrative Review Commitee Member?
Along with EIM reports, reports of Abuse must also be filed with the following entities
What is APS and OAPS?
The failure to protect an individual from health and safety hazards as part of routine care, service provision or as outlined in the ISP.
What is Failure to Provide Protection from Hazards?
An administration of medication fails to occur.
Is the person designated by a provider or SCO who has overall responsibility for incident management
What is the Incident Management Representative?
There are two types of abuse incidents that could be filed under the abuse category involving restraints.
What is Misapplication/Unauthorized Use of Restraint Injury and No Injury
This category of Neglect is to be filed by Supports Coordinators.
What is Passive Neglect?
