This is when you are sent from one professional support to another, due to the new professional being better able to meet your needs.

The most common coping skill people tend to use.
This is a local resource that offers socialization, learning opportunities, exercise equipment and classes, swimming, and more.
What is the Community Center (or Senior Center)?
We start discharge planning at this time.
What is the beginning of treatment?
A word that describes something that helps us move from a less ideal state to a more ideal state (e.g. anxious to calm-minded)
What is a state-shifter?
This program's focus is on making sure seniors and people with disabilities have access to nutritious meals, but they often help with social connection and safety checks as well.
What is Meals on Wheels?
This is what it is called when we cut back on the number of days or hours you are in therapy.

This is a practice of noticing what is going on outside of our mind - may have a focus on the senses.
