Who is the Community Habilitation’s nurse and Where would you find her contact information?

For someone that lives in Residential - What is the earliest and latest an appointment can start?  What is the longest amount of time that someone who lives in residential be supported in 1 day?

The earliest an appointment can begin, is 8am. The latest is can start in 3pm. (Note the latest we schedule is 2:45 to allow for flexibility.) No more than 6 hours. ( It can be broken up into multiple appointments)

You pick up a resident from Clinton St. for their appointment; they would like to attend a birthday appointment at Burkeshire.  They will be able to work on all their goals (Community inclusion by socializing with others there, exercise as they will play basketball in the back yard, learning because they require assistance filling out a birthday card). Can we bill for this? Why or why not?

No, cannot provide services in a certified setting, unless it is for a LP plan or COS.

Where would you find the Staff Handbook?

Online copy on both Share point and in Relias. Updates are typically completed annually where you would then acknowledge on Relias.

While supporting someone in Community Hab, they show you a bruise on their arm. This did NOT happen while during Community Hab, do you need to complete a GER?

A GER must be completed for all injuries whether with a program or not (excluding hangnails and papercuts). Check first that another program has not completed it, if not you must complete to the best of your ability.

What is a LOS and why do we submit them?

Level of Supervision – They are to attest that all supervision levels were maintained for the person supported during their appointment. This varies from person to person. If they are independent, they will not have one.

A community person attends their community Hab appointment and has the following goals, (Community inclusion, learning and safety.) After 40 minutes of brainstorming, they declined all activities offered and they choose to go home. They did not successfully complete any goals. Can we bill for this? Why or why not?

Yes, the goal must reflect everything offered and their response.  Notify your supervisor

Who is responsible for managing your PTO?

YOU - Employees are responsible for managing their own PTO hours. While employees may use all PTO that is accrued, it is recommended that they monitor their usage and balance to accommodate personal needs. Senior management will review PTO use quarterly.

If you are working with someone in community hab and they develop a head ache, can you provide them with medicine you have on hand so they can continue their appointment.

Never is a staff allowed to provide any type of medicine to people supported.

A person supported who lives in the community, attends a full 6 hour day of Community Pre-Voc, can they also receive services in Community the same day? Why or Why not?

Yes, due to them being a community based person, there is no cap on the hours or services they can received in 1 day.

A community person tells you that their neighbor was not able to get groceries this week due to the snow storm. The person supported asked to get their neighbors groceries while with Community hab. The person has a goal for Community inclusion and money management.

No, the program is to support the person.  They can shop for themselves, but staff are not to run errands for people not in the program.

What steps do you take if calling in?

Notify supervisor via teams as soon as possible. Follow instructions provided. If you do not hear back, contact your first person.

A person supported is playing pickleball, a peer misses the ball and hits the person supported on the side of the neck with a paddle, do you need to call the nurse?

Yes! Anything above the shoulder (Neck/Head).  A GER must be completed as well.

What is the window of time that a documentation can be submitted.

Must be submitted within 24 hours of the appointment.

Other forms of exercise offered, how long did they walk, how many steps did they get in, keep an appropriate pace, did they set a goal for themselves and meet it? etc

How much time is a staff require to be off in a calendar week?

24 straight hours in a calendar week. This is why you need to notify your supervisor/manager when you pick up hours outside of the program.

Why would one contact the justice Center and where is one place you would find the  number?

If you witness or have knowledge of an event in which a person supported has been harmed. Hanging in Joannas office, you can google it.

What steps do you take when someone is having a seizure?

STAY with them, keep them SAFE, turn them to the SIDE.

Someone has a goal to improve their health.  During their 8-12 appointment, they decided to go to Dunkins for Breakfast and then McDonalds for lunch. Explain what you would do in this situation?

While the program is person centered, goals can be accomplished at one location.  The timing of appointments allows them to eat both meals at home.  If they choose to go out for a meal, have them pick one while also encouraging healthy choices as well as an exercise activity.

How do you know if you can volunteer at a site?

Cannot take from a paid position.

Where would you find a persons behavior plan and why would you need to know this?

Under “Care” – “Behavior Plan” in Therap. You need to be familiar with your persons behaviors and how to intervene.  Staff are also require to document these behaviors. See Handout for examples
