If a patient is admitted with a wound, what must you do?
-add LDA for each wound found
-place SOS (select A if present on admit)
-upload picture in EPIC on admit AND again at discharge, plus during hospital stay if worsens
-Consult placed to wound care -(ALL new hospital acquired skin issues must have a consult)
*Utilize wound care standing orders until seen by wound care for patient specific orders*
What is our glucose protocol?
If patient is <50 or >400, immediately restick.
If # remains <50 or >400 then treat patient per order, place STAT serum glucose lab and notify physician.
if physician does not want lab you must document in a note!
