Who is exempt from fasting and what must they do instead? 

Children do have to participate in fasting. For adults,  temporarily sick people, women in their menses and post-natal bleeding, pregnant women, and those traveling for non-frivolous reasons are exempt but they must make up their fast on a later date. Those with a permanent illness must pay the fidyah by giving a day's meal for every day of fasting missed or the equivalent in money donation.
