There are 18 members and 3 staff in the gym. Two staff are standing together talking while the third is playing basketball with members. One member leaves the court and sits behind the bleachers.


Challenge: Identify every supervision concern, not just the obvious one.

Staff clustered together, staff distracted by conversation, staff participating rather than maintaining a broad view, member moving to an area with reduced visibility, unclear zones/coverage.

A group of 22 members leaves the classroom for outside. A staff member counts 22 before leaving. Halfway there, one member says they forgot their jacket and runs back. Another staff member says, “They're fine; they know where they're going.” Everyone continues outside.


Challenge: What went wrong, and what should have happened?

This is situational, but the child should never go back alone. One staff member can take 2–3 members with them to retrieve the jacket, ensuring the other staff member remains within ratio. Depending on the situation, the entire group can also pause or stay together until the jacket is retrieved.
