This symbol signifies that a provider will see patients of all ages.
What is a star?
An advanced practice registered nurse that may act as a patient's PCP. Able to write referrals, prescribe medication (not controlled), and order treatments.
What is a Nurse Practitioner (NP)?
A person coming in for the first time ever to an ACCESS location being seen for back pain would fall under this visit type.
Program offered to men and women of reproductive age, who may be insured or uninsured. Program helps to cover costs for preventative exams (physicals, STD testing, birth control, etc).
What is Title X?
This color lets us know that the provider is a pediatrician.
What is green?
Doctor trained to deal with a woman's reproductive health during prenatal and postpartum periods.
What is an Obstetrician (OB)?
A patient who has only seen a primary care provider, seeing an OB/GYN for the first time after obtaining confirmation of her pregnancy from a doctor.
What is an Initial Prenatal Visit?
Program for uninsured female residents of Illinois aged 18-64. Covers screening visits such as: pap smears, clinical breast exams, and pelvic exams.
What is SAC (Stand Against Cancer)?
The phone number that we use to call a health center to ask a question, or transfer a patient to the front desk.
What is the backline / transfer number?
A primary care physician who specializes in the care of children, usually seeing patients from birth up until they are 18.
What is a Pediatrician?
A patient scheduling for their third telehealth appointment with a Behavioral Health Counselor would be put under this visit type.
What is telephone BHC 30 visit?
Employee of ACCESS who helps patients apply for Medicaid, Sliding Fee Scale, or with re-determination of benefits. Many health centers have one, but some do not.
What is a Benefits Specialist?
Doctors still in training at Sinai who are able to see primary, OB, and peds patients.
What are residents?
A provider who specializes in diseases and disorders of the heart.
What is a cardiologist?
This visit type would be used for a patient who HAS NOT been seen for over three years (or not at all), to see a Cardiologist or Infectious disease provider.
What is New Patient Specialty?
Type of insurance plan that requires the subscriber to select a specific Primary Care Physician. Under this type of plan, and ACCESS patient can see any ACCESS doctor, so long as they are assigned to an ACCESS doctor.
What is an HMO?
