What are acceptable identifiers when completing patient ID?
Acceptable IDS are Account #, Medical Record #, Patient last, first name; DOB, Health care number which are Verbally confirmed with two of: Full Name; DOB; Address; Phone number . Visually patients can show ID that match this info
What is Code Yellow used for?
Missing patient; it provides guidelines for defining a missing patient, completing a risk assessment, and coordinating a thorough search on the premises.
Examples of conditions where Code White may be called
1. A person is verbally/physically threatening towards self, staff, patients and/or visitors 2. A person is not responding to verbal de-escalation techniques, negotiating, redirection, limit setting and problem solving techniques by staff 3. A person may require restraint (chemical, physical, env...) and is being or is anticipated to be resistive to restraining procedure 4. Urgent assistance is required

What is a code grey? When is it called?
Code grey is to alert the hospital and respond to an unscheduled/unplanned service infrastructure failure (loss of essential services), an air contamination incident (external/internal) and severe weather. This will allow hospital to provide an immediate plan of action and to continue its clinical and non clinical operations

What is the Suicide Risk Assessment Policy/Procedure?
All mental health patients will be assessed for suicide risk using the SAD PERSONS screening tool (conducted on the first clinical meeting) and/or as clinically indicated based on judgement of clinician; safety plan will be put in place and further assessment/screening will be conducted if needed.
When are ID bands produced? Does Outpatient Mental health use patient ID bands?
Patient ID bands are created after patients have been positively identified by two acceptable patient identifiers prior to any service/medical procedure/diagnostic test/medication admin. OPMH does NOT use ID bands however the medication clinic does
What is a CODE 111 called for? Can we initiate other codes while calling a Code 111?
Code 111 is for "Short Term Assistance"; which is for an immediate response to a situation anywhere in the hospital; it can be activated independently or in conjunction with other Emergency codes.
