What's the difference between a discharge plan and discharge summary?

Discharge Plan I can bill for as a 560 as the client is present and will sign discharge plan.

Discharge Summary I can not bill for it and is a 599 note. I complete discharge summary and sign for client/parent "unable to sign".

Yes. All PLANs in all notes should be specific. Meaning add an objective or assessment that you will be doing with client in next session.

Your ADULT client comes in and says "I no longer want my sister as my emergency contact, I want to have that release voided". What are the steps that need to follow? Do we void the ROI? (void meaning erase the ROI)

1- Ask client for another emergency contact (as adult they need to have an Emergency Contact)

2- Have client provide a handwritten request to revoke ROI 

3-Q/M must document a 560 note (15 minutes) that indicates client is revoking ROI & providing new updated ROI

4-Enter-Letter entered into the binder to be uploaded
