  • Fluid volume overload (TACO) 
  • Hypocalcemia — citrate in stored blood binds calcium. Watch for tingling, muscle cramps/tetany, prolonged QT, dysrhythmias.
  • Hyperkalemia — potassium leaks from stored RBCs over time; large-volume transfusion can raise serum K⁺ → weakness and potentially dangerous dysrhythmias.
  • Hypothermia 
  • Dilutional coagulopathy / thrombocytopenia — massive RBC replacement can dilute platelets and clotting factors, increasing bleeding.
  • Iron overload — more associated with chronic repeated transfusions over months/years; excess iron can accumulate in the liver, heart, and endocrine organs.
  • Transfusion reactions
