Person’s name, date of birth, Social Security number, any care services the individual has
What is confidential information?
A signed authorization form the customer or their legal representative.
What is written consent?
ADRC staff must report to their supervisor any breach in confidentiality as soon as it is discovered.
What is DHS requirement of compliance?
Staff, volunteers, board members and contractors
What is confidentiality policy applies to these folks?
Staff providing OC, I&A, benefits counseling, functional eligibility determination, enrollment counseling and other ADRC services
Who has access to confidential customer information?
Verbal authorization form the customer or legal representative

Annual distribution and review of the confidentiality policy, explanation of types of information considered confidential, signature and non-disclosure agreement

Be informed about the confidentiality policy, see their records and be notified if there is a breach in confidentiality.

Ensure conversations with and about customers are private. Be cognizant of your volume, identifying information when meeting customer in public.
What is safeguards to protect confidentiality?
Never leave your laptop unattended or in an unlocked car. When leaving a laptop in a locked vehicle, be sure it is out of sight and encrypted.
What are staff actions to safeguard the confidentiality of our customer?
Customers should be told that the information they share with the ADRC is kept in confidence and may be shared, when needed, with the customer’s consent.

Income, bank statements, assets

Transferring the long-term care functional screen for enrollment purposes.

Terminate access to customer information and data systems when someone leaves their employment with the ADRC.
What is responsibility of local security officer?
Documents that are no longer needed are shredded.
What is measures to safeguard the privacy of customer records and data?
Reporting abuse/neglect, cooperating with public health, APS, law enforcement, court order and emergency situations.
What are the limits of confidentiality?
