  • People who reside in one of the 28 communities we serve. See list below. HOW MUCH WILL IT COVER? Up to $200 in one-time veterinary care for pets of those in need, once per year.

What qualifies someone to participate in SCO?

SCO Eligibility 65 + 

Beginning January 1, 2026, all SCO enrollees must be enrolled in both Medicare Parts A and B, as well as MassHealth Standard (Medicaid), to remain in a SCO plan.

What qualifies someone for the PCA program?

  • You may apply for the PCA Program if you are eligible for MassHealth Standard or CommonHealth, and you have approval from your doctor for PCA services; a chronic or permanent disability that prevents you from performing your own personal care; and a need for physical (hands-on) assistance with at least two of seven activities of daily living (ADLs) (mobility, bathing/grooming, dressing/undressing, passive range-of-motion exercises, taking medications, eating, and toileting). Before you can begin to use PCA services, you must contact a PCM agency who will need to obtain an authorization from MassHealth for you.

What is the Frail Elder Waiver and what service plan does it qualify consumers to participate in?

The Frail Elder Waiver is a MassHealth Program for participants in the community who are clinically eligible for nursing home placement, but choose to remain in the community and receive services in-home. The FEW has a higher income limit.

Those who qualify for the FEW can participate in the Community Choices Homecare plan that offers increased hours in home for additional support.
