The mayor received a letter from his friend in Petersburg warning him of the arrival of a general Inspector in disguise to inspect his district and as he knows everything in town is in disorder. So the friend warns the Mayor to take all the necessary precautions to put everything in order if he wants to keep what is in hand .

He contradicted the judge and didn't agree with that opinion.

The nightcaps and bed clothes are all worn out. There are lots of dirty patients who all look like chimney sweepers. The patients aren't given any medicines as nobody cares about their lives. They are even allowed to go around the hospital smoking filthy tobacco. The doctor who is responsible for them doesn't speak Russian. The wards are crowded with patients and there are no records for the patients, with their names, the diseases they suffer from, when they are admitted and such things .
