This COSOF is produced when a child turns 6 or is dismissed from services before his/her 6th birthday.

An SLP is expected to fill out this form to record the movement of student folders from campus to campus.
What is the Folder Exchange Record Form?
A parent gives you a "prescription" or note from a doctor or clinic stating that their child needs speech therapy or an evaluation but has never been seen by an SLP or had an evaluation. The SLP should then notify this person(s).
