Life-Threatening Emergency Without 911: A medical crisis occurs (such as a severe seizure or cardiac event) where staff fail to call 911 immediately.

Unexpected Hospitalization: Any sudden, unplanned transfer of a participant from the day program to the emergency room or hospital

What is the procedure for staff absences which impact the Day program transportation schedule? 

  • Day Program Staff must contact (speak to their DP Mgr on the telephone) the Day Program Manager by 6:30am when calling out for a shift.  The DP manager will rearrange the transportation route /coverage (or cover the transportation route themselves).
  • If the Day Program Manager cannot be reached, the DP staff calling out must contact (speak to) the Day Program Supervisor by 6:30am when calling out for a shift. The DP Supervisor will rearrange the transportation route coverage (or cover the transportation route themselves when needed).
  • If the Day Program Manager and the Day Program Supervisor can’t be reached, the DP staff calling out must call their assigned Day Program site at 8am and inform the Day Program Manager of their call out.  The manager will make alternate transportation plans or, provide transportation to the group assigned to that route. The DP manager will reassign DP service recipient grouping and report to their Program Supervisor all changes.

Physical Assault: A severe physical altercation between participants or directed at staff, resulting in moderate to major injury requiring medical treatment.

Answer: Yes, this is an Elopement (or missing person) incident. Any time a participant who requires supervision wanders away and their location is unknown, it must be reported.

Reportable Situations: (include but are not limited to)

  • Any incident listed on the Department of Human Services Unusual Incident Category List
  • Staff who are 15 minutes or more late for their scheduled shift or who leave shift early.
  • Staff ending shift 15 minutes or more after scheduled shift ended.
  • Staff call outs and/ or staff coverage issue (i.e.- gap in hours of coverage, lack of adequate coverage, more staffing than needed)
  • Staff on the job injury or illness, medical emergency, death
  • Staff inappropriate behavior at work (i.e.- sleeping, illegal drug or alcohol use, behavior causing concern as if under the influence of an intoxicating substance, sexual harassment, discrimination, workplace violence)
  • Staff exposure to blood or other potentially infectious material
  • Police contact for any consumer or staff issue.
  • Alleged rape or sexual assault of staff
  • Any fire or fire safety issues including inoperable smoke detectors, fire alarms, or fire extinguishers.
  • Missing petty cash over $5.00 and/ or missing gas card
  • Any amount of money stolen or missing from a consumer or staff
  • Unauthorized visitors to the program (this includes current employees not scheduled to work, as well as past employees).
  • Operational Breakdowns (vehicle issues presenting safety or health hazard, lack of transportation available for appointments or outings, need for tow truck, loss of utilities such as electric, heat, telephone, potential hazards to residents and staff such as snowy or icy walkways that cannot be cleared, GH driveways that have not been plowed).
  • Any medication issue as outlined in P & P’s 3-3, 3-4, 3-5, 3-6, 3-7, 3-16.
  • Any emergency restraint
  • Staff bringing prohibited personal items to work (i.e. pillows/blankets, space heaters, laptops, tablets, schoolwork, books, gaming systems, etc.).                                
  • Missing or expired agency vehicle registration, insurance, or inspection sticker    
  • Staff who are transferred or pulled to a program with single coverage, with individuals who have BSPs, and the transferred/ pulled staff have not completed BSP training via STO with a qualified trainer or training with behaviorist. Without BSP training, staff cannot work alone.
  • SAF not available for shift.
