Fill out a new ROI. Some facilities are more particular than others when it comes to ROIs. If there are any scribbles, or markings on the ROI without the proper initials next to it, they could deny the ROI; therefore potentially prolonging care.

True. Once a document is signed off, it can be appended, but it cannot be deleted.
A 20-year-old patient comes into the clinic with their father for a chiropractic appointment. Once they arrive, they are told that they need to update their annual paperwork. Who signs off on the annual paperwork? The 20-year-old child or the father?
Since the patient is over the age of 18, they would sign off on the annual paperwork themselves. If the patient is not their own guardian (does not have guardianship over themselves) then their guardian would sign off on their behalf. However, the proper documentation for guardianship would also need to be documented.

What is the difference between a phone note and a flag?
A phone note (once signed off on) it becomes part of the patients chart, therefore a legal document. A flag, is more like an FYI or informational and are not saved within the patients chart.
