What priority level would you assign the following incident? Who should be alerted?
You arrive to the lab one morning and see you are scheduled to work in transfers. You arrive in your area and note that the PCR machines are turned off which strikes you as odd but you don't think twice and turn them back on. Once on you realize that the sample runs from the previous night never completed and that the machine alerts you that it lost power at some point in the last night. You alert your shift lead thinking the building just lost power overnight and they prompt you to rerun the samples. You do as told and set samples up to rerun. Later that day you approach the PCR machine because you think the run time should have passed. You notice again that the run did not complete and the machine states that it has lost power only this time you are concerned because you have been in the lab all day and the power never went out. You again notify your shift lead and the two of you realize you will not be able to get the client results in time as the machines are not working.
What is a priority level 2 (or priority level 1 b/c when in doubt escalate to next level)? Who is site level management and director level associate via phone call or text?
This issue poses serious risk to Deibel and the client if not properly addressed. We have alternative methods we could run samples on with client permission but we would want to notify a director to help us figure out how best to notify client since there will be a delay in results and a new method listed on COA. Also, some clients may need results in a certain method and a director can help coordinate with another lab if samples need to be redirected until PCR machines were fixed which a director could help coordinate.
