AHH has an inverted Organization Chart that shows our programs, identifies supervisors of each department/service, identifies the Management Team, the Senior Management Team, and demonstrates our philosophy that people we support are first.

Since we do not provide residential services, we are not able to provide these items for the people we support at all times. What AHH does do is make sure people are linked with the resources they need for daily living activities and other needs.

Even with permission, AHH provides only the information that is relevant. Consents should be limited to certain pieces of information, for a particular purpose and be in effect for a specified time periods. People can limit what info anyone knows and shares about them. In situations where their wishes cannot be respected for legal reasons or because of concerns related to professional ethics, the reasons are explained to them.

AHH does its best to repair, maintain, and replace furniture and equipment on an as needed basis. AHH maintains the physical environment at its offices and assists people we support with this concept for maintaining their own homes.

People have a right to their medical records. However, much of what is contained in the medical record is very sensitive and may not be processed by a client without a negative impact on them. We limit what we will release without a court order, to the CCA and PCP. The CCA can only be released after the information contained in the CCA is reviewed with the client by a licensed clinician.

People who experience a disability, mental illness or other conditions frequently depend on human service organizations for shelter, food, clothing work, transportation and other forms of support. Dependence on the organization often links changes in people’s lives to organizational changes. AHH provide continuity and security for people through the service process. Aspects of AHH operations, such as staffing turnover, can affect the stability and continuity people experience. AHH considers how change impacts people’s continuity and security and acts to minimize its emotional effect on people.
