Safe Work Permits
The personnel assigned to a department who operate the equipment and initiate the Safe Work Permit.
What is the equipment owner
The Shift Supervisor, or alternate, of the unit where the incident occurs. Plant safety professionals may fulfill this role when required
What is the On-Scene Commander (OC)
The uncontrolled distance travelled before a worker hits the floor, ground, or before the fall arrest equipment activates.
What is Free Fall distance?
Defined by repetition, lifting, awkward postures, etc.
What is Ergonomic Hazards?
Placement of a locking device on an isolation point to ensure the equipment / energy being controlled cannot be operated/released until device is removed.

Any special instructions or requirements not listed on the permit form should be written here.
What is the comments section
The designated personnel assigned by their department to respond to all plant emergencies.

An equipment owner must be present when conducting when performing this step.

A designated location where the Incident Management Team (IMT) convenes to conduct emergency management operations.

What is a Personal Fall Arrest System
Hazard defined by the person supplies the force needed to injure him/herself. That is, hit or walk into something
What is Struck Against (SA)
A brightly colored, weather resistant material which is used to identify isolation slip blinds, blind flanges, and spectacle (figure-8) blinds.
What is a blind flag?
Work performed by the equipment owners which does not require Work Order or Safe Work Permit.
