Scenario: An individual comes in with a bottle of medication that is labeled "Risperidone" with the dosage 1 mg by mouth at lunchtime. The bottle has the individual's name on it, but the date prescribed on it is 2 years old and the expiration date is a year old. The description of the pill on the bottle reads "Circle - White". However, when you open the bottle, you observe oval shaped pills that are blue in color. Do you pass the medication? What is wrong with this scenario and what do you do?

DO NOT PASS THE MEDICATION

The bottle is not labeled correctly - the date the prescription was filled and expiration date is outdated. We have to have an updated bottle and prescription for all medications.

The appearance of the pill as labeled on the bottle is not the same as the pill inside the bottle - you do not know what that medication is. You cannot pass a medication that does not match the bottle.

If this happens, contact your PC or PA and they will contact the individuals family, guardian or residential provider to get the situation worked out.
