What is case plans must include behaviors that need to be observed during parenting time to transition to a less restrictive parenting time plan. Information gathered from parenting time helps inform permanency planning decisions such as whether conditions for return have been met, whether the parent’s CPC’s have enhanced and if positive behavior changes have occurred and areas that still need support.

Why transition plans are included in the case plan

What is, the focus of a transition plan for young people is to outline their goals and the steps involved in achieving these goals. Transition Plans are part of the case plan and are youth-centered and youth-driven, and provide services and opportunities to support all youth to make sound life decisions and develop a sense of achievement, usefulness, belonging, and empowerment. 

Complete the Transition Plan for all children age 14 and older, and for young adults age 18 and older participating in continued care through a voluntary agreement.

Case plans for children in OOH care (as applicable based on age and permanency goal) shall include type of living arrangement, the child’s needs, supports, and services, education plan and educational stability, health plan, transition plan, family contact plan, concurrent planning activities if needed, and steps to finalize permanency.

The purpose of the case plan staffing

What is to develop a shared understanding of the reason for DCS involvement and permanency goal. The staffing helps to identify strengths the family, prospective adoptive family, or young adult has and identify supports to address concerns.

How engaging parents in the exploration stage supports case planning 

What is during the exploration stage, the DCS Specialist identifies family strengths, supports, and existing protective capacities that contribute to child protection. Using a strength-based approach, the DCS Specialist asks the parents to identify strengths about themselves as individuals and in their caregiving role, share and discuss their protective capacities, seeking the parent's assessment of their own protective capacities, and exploring their readiness for change and self-determination.

The reassessment of the case plan should determine

What is desired behavioral changes have been achieved, meaning previously diminished caregiver protective capacities have been sufficiently enhanced;

the same services and supports shall be continued; services and supports shall be changed; or no available service or intervention will enable the parent to adequately address the safety threats within a time frame that meets the needs of the child, and a change in permanency goal should be considered.

Reasonable Candidacy has these components 

A child is residing with a parent/guardian, at serious risk of removal from the home. Reasonable efforts to prevent removal are being made by providing safety services as part of a safety plan and/or treatment services to enhance parental protective capacities and, if removal is necessary, the child will be placed in foster care, NOT, a detention facility, behavioral health in-patient or hospital
