This document outlines the responsibilities and requirements providers must follow to participate in the School Readiness Program.
What is the Statewide School Readiness Provider Contract?
Providers are required to accurately record this daily for each School Readiness child and submit no later than the third (3) business day every month.
What is attendance?
Coalitions conduct these to ensure providers comply with contract requirements.
What are monitoring visits?
Providers must comply with all applicable health and safety requirements established by this state agency.
What is the Florida Department of Children and Families?
Overpayments identified by the Coalition may require providers to do this.
What is repay funds?
Providers must keep this information current with the Coalition, including phone number, address, and ownership details.
What is EFS MOD provider portal?
Attendance records must be maintained for this minimum number of years.
What is five (5) years?
When deficiencies are identified, providers may be required to submit one of these outlining corrective steps.
What is a corrective action plan?
Children cannot be subjected to treatment that is severe, humiliating or frightening and it also must not be associated with food, rest, or toileting.
What is discipline?
This assessment tool measures classroom interactions between teachers and children.
What is CLASS?
Screening for each child, age six (6) weeks to sixty (60) months, within forty-five (45) calendar days after the child's first or subsequent enrollment in the SR Program to identify a child who may need intervention practices.
What is a Developmental Screening or ASQ?
Providers must ensure parents sign this document daily acknowledging their child is present.
What is the Sign-in/Sign-out Form?
Submitting false attendance records may be considered this type of activity.
What is fraud?
Safe sleep practices are required for infants to reduce the risk of this condition.
What is Sudden Infant Death Syndrome (SIDS)?
A score of four (4) or higher deems you eligible to contract with the Coalition.
What is Contract Minimum Threshold for contracting?
Provider, officer, or board director listed on the United States Department of Agricultural National Disqualified list.
What is a person not eligible to contract with the Coalition?
PROVIDER agrees to abide by this for each at-risk child under the age of school entry who is enrolled in the SR Program.
What is the Riyla Wilson Action?
Failure to comply with the Provider Contract may result in corrective action, termination, or this five-year consequence.
What is revocation of eligibility to contract?
Providers must immediately report suspected abuse, abandonment, or this.
What is neglect?
Providers may receive coaching and technical assistance to improve this.
