You’re working a late afternoon shift at one of the youth program sites. A 15-year-old participant asks to use the washroom and doesn’t return after 10 minutes. You knock—no response. You unlock the door with another staff present and find the youth slumped over, unresponsive, with shallow breathing and blue lips. 1. Ensure Personal Safety
Quickly assess the scene for hazards (e.g., needles, drugs).
Do not touch any unknown substances.
If safe, proceed. If not, wait for emergency personnel.
2. Check for Responsiveness
Try to rouse the youth by calling their name loudly.
Perform a sternum rub (knuckles rubbed firmly on the chest).
If no response and they appear to be overdosing (blue lips, shallow or no breathing, unresponsive) — assume it's an opioid overdose.
3. Call 911 Immediately
Even if you plan to administer naloxone, you must call emergency services.
Clearly explain: youth is unresponsive and may have overdosed.
4. Administer Naloxone
Follow the directions in your nasal spray kit (or injection, depending on kit).
Administer one dose
Begin rescue breathing or CPR if trained and needed.
If there's no improvement in 2-3 minutes, give the second dose if available.
5. Stay with the Youth
Lay them in the recovery position (on their side) if breathing improves.
Monitor closely and reassure them if they wake up (they may be confused or agitated).
Wait for EMS and provide them with all information.
6. Report & Document
Notify your supervisor as soon as possible.
Fill out an incident report using your organization’s process.
Do not speculate in the report—stick to facts (e.g., “found youth slumped in washroom, shallow breathing, administered naloxone”).
If applicable, report to child protection services depending on your duty to report guidelines.
