A benefit in which members can use after an eligible inpatient discharge after a hospitalization.
What is the post-hospital meal program benefit?
The tab in PW where you can update health screenings such as the OMW, BCS, COL and flu vaccination.
What is the Health Maintenance tab?
The source of truth for this assessment is in the PW banner.
What is the HRA/RSA?
A benefit that must be stated on each initial/welcome call and/or when a member mentions triggers such as: lack of physical activity, weight gain, etc.
What is the Renew Active and Get Moving challenge benefit?
The phrase that must be typed in the additional comments section after the member gives you a date of a health screening such as the: OMW, COL or BCS.
The only assessment that requires approval from the member prior to starting this assessment.
What is the HRA/RSA?
A platform that assists Care Navigators find resources. The resources given from this platform are known as CBOs.
