Infection Prevention
This type of PPE is required during high-contact care activities.
Enhanced Barrier Precautions (EBP)
This should be addressed within 3-5 minutes of initiation. Failure to do so often results in frustration, decreased feelings of trust and increased workload. Being proactive often reduces the frequency this is used.
Call lights
This SELF PERFORMANCE code involves verbal cues and/or touch.
Supervision
Resident completes activity by themselves with no assistance from a helper.
Independent
Best practice is to bag items at the source, keep clean items at least 10 ft away from soiled items, don't allow clean items to come in to contact with clothing, don't place soiled items on floors, chairs or tables, and bag soiled items for transport to the soiled holding area.
Linen Handling
This should be completed at the end of every shift...not the beginning, not the middle, not the next day.
Charting/Documentation
Helper does LESS THAN HALF the effort. Helper lifts, hold, or supports trunk or limbs, but provides less than half the effort.
What is Partial/Moderate Assistance
Helper sets up or cleans up; resident completes activity. Helper assists only prior to or following the activity. (Cut up meat, open milk carton, lock wheelchair wheels, hand them a cane/walker)
What is setup or clean-up assistance
This must always be done before entering and upon leaving a resident's room, prior to and after wearing gloves, whenever hands are visibly soiled, before eating and after using the restroom.
Hand Hygiene
This should be documented as soon and as accurately as possible. Accurate documentation helps with early identification and prevention of problems including weight loss, dehydration, infection and wounds.
Meal Intake
A hospice resident is in need of a bath/shower; who is responsible for this task?
The facility CNA
Helper does ALL of the effort. Resident does none of the effort to complete the activity OR the assistance of 2 or more helpers is required for the resident to complete the activity.
What is dependent
This infection requires use of PPE including gown and gloves, requires hand washing with soap and water, and bleach-based cleansers for disinfection.
C. Difficile
This should never be worn in hallways, as the halls are considered "clean" areas. It should be removed while still in resident's room and placed into a bag for transport to the soiled utility area.
PPE
Although the ADL name is misleading, this is where incontinence care is documented. It doesn't matter what device resident uses for elimination (brief, toilet, bedside commode, bedpan or urinal.)
Toilet Use
This is the magic number to determine if the helper provided Partial (moderate) or Substantial (maximal) assistance.

This infection requires use of PPE including gloves, gown, N95 mask and face shield or goggles.
Covid 19
Residents are expected to receive this as care planned. At least once a week. It is their right, their preference, and it's required!

What is Substantial/maximal Assistance.
Resident requires 2 helpers to complete an activity, even if they are able to do part of the work themself.
