Sally, our newest resident, is under my care today. I answer her call light, promptly, and assist her into getting out of bed. Since I am not sure how she performs bed mobility, I assist her with pulling back the covers, and guide her legs out of bed to the floor. Rebecca has her call light on the use the toilet. She is waiting in there when I arrive. Rebecca is unable to lock her brakes prior to transferring to the toilet due to right side limited ROM. I assist her by locking her brakes and then leave because her limited ROM does not affect her ability to transfer and toilet herself. Code support for toileting.

Marla is at it again. She tends to have really good mornings and then after lunch and her nap, she is up and going. Marla is yelling out "hey, I didn't get no lunch" and stops every one she sees to ask if they can get her something to eat. Lucy, the CNA on shift, tells Marla she will get her something to eat but "you don't need it, you just ate lunch" and leaves Marla in the hall yelling "what did you say?". Stella is 98 years old. She goes to breakfast in her w/c. On her way back, she flags down Bob and asks if she can go for a walk. Bob is happy to help her and grabs her walker and gait belt from her room. Stella is tired from her bath and breakfast so when she attempts to stand up, she is unable to without Bob's assistance. Bob has to provide weight bearing support to stand. Once she is standing, Stella uses her walker and Bob holds onto the gait belt for support/balance. Code ambulation.
