The nurse administered prn medication in response to the pts agigitation. The medication was effective in reducing his anxiety & agression.

The MRN had a meeting with CN & explained that they did their best to keep Mr. Garrison safe by providing observation rounds, bed alarms, bed rails, and medication to reduce his anxiety, but did not anticipate that he would climb over the railings. Discussion in place to update care plan.

Due to Mr. Garrison’s high risk of falls, nurses adapted the environment & began to use bed alarms, bed railings, and other staff to stay around the area to re-orient him back to bed.

The bed alarm goes off & nurses began to respond, but by the time they got to the room, Mr. Garrison had fallen on the floor.
