Nurse hears the bed alarm go off and responds immediately

Nurse reported the pts increased confusion, refusing some of his medications, eating & drinking less, less patience for tasks, increased need for re-orientation and redirection.
2.11 Adheres to the duty to report.
The MRN was worried about placing the patient in restraints as they admitted they did not like the idea of restraining someone.
7.6 Advocates for safe, competent, compassionate and ethical care for clients.
The nurse administered prn medication in response to the pts agigitation. The medication was effective in reducing his anxiety & agression.
1.11 Applies knowledge of pharmacology and principles of safe medication practice.
The MRN had a meeting with CN & explained that they did their best to keep Mr. Garrison safe by providing observation rounds, bed alarms, bed rails, and medication to reduce his anxiety, but did not anticipate that he would climb over the railings. Discussion in place to update care plan.

Due to Mr. Garrison’s high risk of falls, nurses adapted the environment & began to use bed alarms, bed railings, and other staff to stay around the area to re-orient him back to bed.

The nurse utilized bed alarms, bed railings, increased observation, 1:1 care, prn medication,

The MRN acknowledges when they gave medications to sedate the patient, different restraints may have been warranted, or speaking to someone regarding the new behaviour Mr. Garrison had developed.

incidences, near misses, and no harm incidences.
The bed alarm goes off & nurses began to respond, but by the time they got to the room, Mr. Garrison had fallen on the floor.
