When is the employment readiness assessment completed and why is this important?

The Employment Readiness Assessment is completed during the intake process. It serves as a tool for staff to gather information about the consumer's current abilities and determine the level of support they may need in various areas. (ex. Physical Assistance – Staff provides hands-on support to complete the task. Partial Physical Assistance – Staff provides limited hands-on support, with the consumer performing part of the task independently. Verbal Prompts – Staff provides spoken cues or reminders to help the consumer complete the task. Independent – The consumer is able to complete the task without any assistance.

General Conservatorship : For adults who are unable to care for themselves or manage finances. The conservator may have authority over: Healthcare decisions, Housing, Daily care, Finances and assets

Medically Conserved: A person is medically conserved when the conservator has authority to make medical decisions on their behalf. This is often part of a Limited Conservatorship or General Conservatorship where medical decision-making powers are specifically granted.

What does POS stand for? Who do we reach out for to receive this document and why do we need this document? 

A Purchase of Services (POS) authorization is required before initiating certain services for a consumer. To obtain this authorization, staff must contact the consumer’s Service Coordinator. We request this document for the purpose of securing approval and funding for specific services that support the consumer’s individual program goals. Having an approved POS ensures that services are delivered in compliance with funding requirements and the consumer’s Individual Program Plan (IPP).

The CDER (Client Development Evaluation Report )  is a structured assessment tool used to track the development, functioning, and support needs of individuals receiving services through California Regional Centers. It is typically completed by service providers or Regional Center staff and is used for individuals with developmental disabilities. Purpose: To document a client’s current level of functioning in various domains (e.g., self-care, communication, mobility, social skills).To identify support needs and monitor progress over time. To support service planning, funding decisions, and resource allocation. To help in statewide data collection and policy planning.
