What is confirm the contact is the same person (via address, phone number, DOB or email), confirm the spelling of contact's name; inform the contact that they were exposed more recently and that their new qt dates are 03/07 - 03/17; cross-reference the contact IDs in both the records and close the prior exposure record as "duplicate entry"?
CT Workflow - Jane is an Onondaga Contact who is currently experiencing a cough, slight fever and a runny nose. She attributes the runny nose to allergies. Her current Quarantine dates are 04/05 - 04/15. She lives alone without a yard and has a dog.
What is extend Jane's QT end date to 4/19 to reflect a total of 14 days. Recommend that she get tested and encourage her to start isolating. Inform her that someone from the LHD will be reaching out. Notate CSS need for help taking care of dog in the record, re-issue revised quarantine notice, and convert status to PUI. Then engage your supervisor for CSS needs?
CT Workflow - A contact residing in Onondaga was reached for an initial interview and signed up for daily SMS follow ups. On day 3 of daily follow ups the contact stops responding to SMS and cannot be reached.

CT Workflow - Paul received his 1st dose of Pfizer vaccine on 03/01/21 & his 2nd dose on 03/15/21 in Onondaga county. Paul's exposure date is 04/04/21. Paul disputes his need to Quarantine because he is vaccinated and has no symptoms.

At the bottom of the interview, select "partial interview, needing call back" and select "No" when asked if vaccination has been verified. Engage your supervisor for NYSIIS Vaccine Verification Channel assistance. Once verified, add a note in the record, select "Yes" at the bottom of the interview when asked if vaccination was verified and "Yes" to close record as "Vaccinated, exempt from quarantine".
